External DCR (Tear Duct Surgery)

External DCR is a well-established surgical procedure designed to restore tear drainage by creating a new pathway from the tear sac into the nose.

It is a highly effective treatment for persistent watery eyes caused by tear duct obstruction.

What Is External DCR? 

External DCR (dacryocystorhinostomy) bypasses a blocked tear duct by forming a new drainage channel. 

It is used to treat: 

  • Blocked tear ducts (nasolacrimal duct obstruction)  
  • Chronic watery eyes (epiphora)  
  • Recurrent tear duct infections (dacryocystitis)  

By restoring drainage, the procedure resolves persistent tearing and reduces the risk of infection.

Suitability

External DCR is recommended when tearing is clearly related to a structural blockage of the tear drainage system. 

It is often considered where long-standing symptoms or recurrent infections significantly impact quality of life.

The Procedure

The procedure is performed under local anaesthetic with sedation and typically takes around 45 minutes.  

A small incision is made on the side of the nose to access the tear sac, and a new drainage pathway is created. A fine silicone stent is placed temporarily to support healing.

Recovery & Aftercare

  • Mild bruising or swelling around the incision site is common and settles within 1–2 weeks  
  • Activity is limited initially, with gradual return to normal over several weeks  
  • The silicone stent is removed after 2–4 weeks  

Follow-up ensures the new drainage pathway matures and functions effectively.

Outcomes

External DCR has a high success rate of approximately 95%, offering reliable, long-term relief from tearing.  

When appropriately selected, patients experience a significant improvement in comfort and day-to-day function.
