Endoscopic DCR (Tear Duct Surgery)

Endoscopic DCR is a minimally invasive procedure designed to restore normal tear drainage by bypassing a blocked tear duct.

It offers a refined, incision-free approach to treating persistent watery eyes, with excellent functional outcomes and minimal visible signs of surgery.

What Is Endoscopic DCR?

Endoscopic DCR (dacryocystorhinostomy) creates a new drainage pathway between the tear sac and the nasal cavity, allowing tears to flow normally again.

It is used to treat:

  • Blocked tear ducts (nasolacrimal duct obstruction)
  • Chronic watery eyes (epiphora)
  • Recurrent tear duct infections (dacryocystitis)

Rather than addressing the obstruction directly, the procedure bypasses the blocked duct entirely — restoring effective tear drainage and improving day-to-day comfort.

Why Endoscopic?

Traditionally, DCR surgery involved a small incision on the side of the nose. With modern techniques, the procedure can now be performed entirely through the nostril, using endoscopic instruments.

This approach offers several advantages:

  • No external incision or visible scar
  • Minimal disruption to surrounding tissues
  • Reduced swelling and bruising
  • Faster recovery

For many patients, this represents the most refined and cosmetically discreet approach to tear duct surgery.

Suitability

Endoscopic DCR is recommended for patients experiencing persistent tearing due to a confirmed blockage or narrowing of the tear drainage system.

Careful assessment is essential, as tearing can have multiple contributing factors. The procedure is most effective when the cause is clearly related to outflow obstruction.

The Procedure

Endoscopic DCR is typically performed as a day surgery procedure under general anaesthetic

During surgery:

  • A small opening is created between the tear sac and the nasal cavity
  • fine silicone stent is placed to maintain the new drainage pathway during healing
  • The procedure is completed without any external skin incision

Surgery usually takes approximately 45 minutes, with a strong emphasis on precision and long-term function.

Recovery & Aftercare

Recovery is generally straightforward and well tolerated.

Following the procedure:

  • There are typically no visible external signs of surgery, such as bruising or swelling
  • Mild nasal congestion or discomfort may be experienced for a short period
  • Eye drops and nasal sprays are prescribed to support healing
  • Strenuous activity is avoided for the first week to reduce the risk of bleeding

The silicone stent is usually removed 2–4 weeks after surgery, which is a simple and painless process.

Outcomes

Endoscopic DCR is highly effective, with success rates of approximately 95% in appropriately selected patients.

Most patients experience:

  • Resolution of watering
  • Improved comfort
  • Reduced risk of infection

As tear drainage is a complex system, careful diagnosis and surgical planning are essential to achieving the best outcome.

A Considered Approach to Treatment

Endoscopic DCR represents the modern standard in tear duct surgery – combining precision, minimal invasiveness and strong functional outcomes.

For patients with persistent watery eyes, it offers a reliable and discreet solution that restores both comfort and confidence.
